HACCPYP Service Delivery

⁠⁠⁠The City of Whittlesea (“the Council”) seeks to engage the services of a suitably qualified and experienced contractor (“the Service Provider”) to provide the titled Services.

Scope:

The City of Whittlesea is seeking a Contractor to provide services as part of the Home and Community Care Program for Younger People (HACC-PYP). The Contractor must comply with the HACC Program Manual and associated standards.

The Contractor must provide one or more of the following services:

• Domestic Assistance (DA)

• Personal Care (PC)

• Carer support / Respite Care

• Assistance with Meal preparation

• Escorted/unescorted shopping

•Transport

The Contract is conditional on Council receiving State Government (HACCPYP) funding for the provision of the above service types. Any changes to the funding arrangements may require contract re-negotiation which may consist of revised service levels, or termination of all contracted services.
