Tender Description
GCHHS is seeking innovative solutions from suitably qualified organisations to address the delayed discharge of older patients from hospital who no longer require active medical care but are unable to access timely residential aged care placement.
GCHHS is seeking market input on medium to long term solutions that increase access to appropriate residential aged care settings. Solutions may include infrastructure provision, service delivery, integrated models or other innovative approaches.
