Introduction
The Central Coast Local Health District (the CLHD) invites registered Community Housing Providers who have a presence in the Central Coast area to submit a proposal to provide compliant, safe and responsive medium-term transitional accommodation for eligible Mental Health consumers, which they feel delivers the best quality service best value for money, and meets all mandatory Policy Directives.
Scope
The successful Housing Providers will:
• Provide medium-term transitional accommodation (up to 26 weeks per referral);
• Deliver tenancy management and support services;
• Work collaboratively with CCLHD Mental Health Services and the Homes for Recovery Clinical Coordinator
• Support consumers to transition into long-term sustainable housing outcomes.
