Background
The General Practice in Aged Care Incentive (GPACI) Project ("Project") is designed to trial supporting general practice teams in improving coordination of care between aged care residents living in RACH and the general practitioner leading to a more sustainable and satisfied general practice workforce.
The Project services are to be delivered for a period of 10 months across 2025 - 2027 financial years.
The Project aims to enhance the experience, efficiency, and clinical effectiveness of GPs providing care within RACHs to support general practice teams to utilise GPACI effectively, increase workforce capability utilising effective care modality and support coordination of care between aged care residents, the general practitioner, ultimately leading to better resident outcomes and a more sustainable and satisfied general practice team.
The Project forms part of a broader primary health care reform, under the Strengthening Medicare Measure - MyMedicare that provides a short-term funded opportunity to engage with Aboriginal Community Controlled Health Organisations (ACCHOs) or General Practices, and Residential Aged Care Homes (RACHs) to support older people to receive quality care as part of the GPACI.
Through this request for tender, Adelaide PHN is seeking to commission services that align with the strategic focus areas of Health Equity and Co-creating solutions.

The Adelaide PHN is one of 31 independent organisations nation-wide funded by the Australian Government to coordinate and improve primary health care services in their local areas.
The Australian Government's objectives for Primary Health Networks (PHN) are to:
increase the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes; and
improve coordination of care to ensure patients receive the right care in the right place at the right time.
Adelaide PHN reaches from Sellicks Hill to Angle Vale and from the foothills to the sea. It encompasses a community of some 1.2 million people.
Adelaide PHN aims to improve health outcomes for the community by commissioning services to address local needs. The Adelaide PHN is guided by community, clinical and stakeholder input.

PROCESS: Potential applicants should read the Adelaide PHN General Practice in Aged Care Incentive (GPACI) Request for Tender Guidelines, which is available once registered. The guidelines contain information on the service specifications and application process.
