Primary Care Impact Grants Program Evaluation

Healthy North Coast is seeking a qualified provider to evaluate the Primary Care Impact Grants program. The evaluation will examine how effectively the small grant, locally driven commissioning model supported multidisciplinary primary care, and how this approach compares with larger, condition- or geography- based PHN grant models.

The evaluation will assess the full grant lifecycle -including design, assessment and award processes - to understand how flexibility, innovation and locally tailored solutions were enabled, and to identify strengths, limitations and unintended consequences for equity, workload and provider engagement. It will also analyse health and system impacts achieved by funded projects, with a focus on chronic disease prevention and management, collaboration between primary care providers, and the ability of small grants to meet community needs.
