Healthy Towns – Link Worker and General Practice Support

Healthy North Coast invites suitably qualified and experienced organisations to submit a proposal to deliver the Primary Care and Link Worker Support Services component of the Healthy Towns initiative.

Healthy Towns is a place-based, whole-of-community program designed to support healthy living and ageing across participating towns. A key element of the initiative is the implementation of social prescribing through dedicated Link Workers embedded within General Practice. Healthy North Coast is seeking to commission a "wrap-around services" provider to support participating practices to establish, implement and sustain effective social prescribing services, including workforce development, systems integration, and contribution to program evaluation.

We are seeking organisations that can demonstrate:

• Experience in primary care, community health, or social prescribing models
• Capability to support multi-site implementation
• Strong governance and financial sustainability
• Capacity to contribute to monitoring, evaluation and continuous improvement
