Background
COORDINARE invites proposals for a person-centred community navigation service that provides an accessible, trusted community front door, builds local capacity, and supports older adults and carers to live well and independently. The service will support early planning and connection to trusted health, aged care and wellbeing services. It is not intended to replace or replicate existing navigation, social prescribing, or care coordination programs, but to complement and strengthen them.
Target population: Older adults and carers aged 65+, Aboriginal and Torres Strait Islander people aged 50+, and all adults aged 45+ who may benefit from early planning and support.
Funding
A total of up to $640,000.00 (ex GST) will be dispersed through this initiative. A single funded project will receive a maximum allocation of $640,000 (ex GST) over the life of the agreement. Program supported under this initiative will run for a duration of 2 years & 2 months, commencing from May 2026 and concluding 30 June 2028.
