About the Request for Proposal (RFP)

WentWest, the Western Sydney Primary Health Network (WSPHN) is seeking qualified and experienced organisations with the capability and capacity to deliver a local Multidisciplinary Team (MDT) service, that meets the intentions of the program announced as part of the Building a Stronger Medicare package and aligned to the needs of Western Sydney.

WentWest's ongoing assessment of health needs and service availability across our region has identified the LGAs of Blacktown and Cumberland as most aligned to this program. This is based on:

• the number of small and solo general practices
• the quantity and range of chronic conditions experienced by the community
• Blacktown LGA: Asthma, Congestive Cardiac Failure, Diabetes, and Chronic Obstructive Pulmonary Disease
• Cumberland LGA: Angina, Diabetes, and Chronic Obstructive Pulmonary Disease
• the impact of sociodemographic factors on health and wellbeing

Organisations are invited to submit a Proposal for one, or both LGA locations in line with their organisations service footprint, capabilities, and proposed model of service delivery.

This RFP is seeking responses from organisations who will act as the contracted lead agency for the MDT(s). The PHN encourages lead agencies to consider joint ventures, partnerships and sub-contractor relationships with individual traders operating within the identified LGA/s. Single discipline organisations are encouraged to seek partnerships with other organisations.
