Tender Description
Some older Australians are entering aged care earlier than they may otherwise need to due to a lack of support for healthy ageing or ability to manage their chronic conditions in the community. This Early Intervention Project - Phase 3 seeks to support older Australians living in the Adelaide PHN to live well in the community through enhancing understanding, awareness and skills of the local primary health care workforce.
The Project also aims to maximise the implementation of evidence informed early intervention initiatives promoting healthy ageing to foster sustainability. Multiple modes of commissioned activity used during phase 1 and 2 of Early Intervention projects has resulted in an increase in workforce capability for general practice, uptake of QI tools and improved quality of life for older people in the Adelaide PHN region. Adelaide PHN aims to continue to promote healthy ageing by utilising health assessments including advanced care planning, social prescribing and increasing nursing capability as identified QI activities.
In addition, it is anticipated that the Early Intervention Project - Phase 3 will enable the continuous support of general practice training and development, quality improvement toolkits and capacity building activities for health professionals across the Adelaide PHN region via Extension for Community Healthcare Outcomes (ECHO)s. These would help to strengthen the role of the primary care sector in reducing the burden on the tertiary health system and provide equitable access to early intervention services when and where it is needed. Through this request for tender, Adelaide PHN is seeking to commission services that align with the strategic focus areas of Health Equity, Partnerships, Data Insights and stories and Co-creating solutions.
The Project services are to be delivered for a period of 18 months across 2025 - 2027 financial years.

The Adelaide PHN is one of 31 independent organisations nation-wide funded by the Australian Government to coordinate and improve primary health care services in their local areas.
The Australian Government's objectives for Primary Health Networks (PHN) are to:
- increase the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes; and
- improve coordination of care to ensure patients receive the right care in the right place at the right time.
Adelaide PHN reaches from Sellicks Hill to Angle Vale and from the foothills to the sea. It encompasses a community of some 1.2 million people.
Adelaide PHN aims to improve health outcomes for the community by commissioning services to address local needs. The Adelaide PHN is guided by community, clinical and stakeholder input.
